Please visit https://covid-19.ontario.ca/assessment-centre-locations/ to find the select pharmacies for the testing of individuals who do not meet the criteria above.

If you meet the above criteria, please fill out the pre-registration form below. Please arrive at the beginning of the time slot you select. If you do not show up during your time slot, you may be turned away. If you are coming with other people to be tested, everyone who requires a test must fill out a separate pre-registration form; staff will do their best to accommodate group testing even if you are booked in different time slots.

Patients who have exhausted all other transportation options to get to their booked appointment can call the Waterloo Region transport booking line: 1-855-786-2674. This Phone Service will run 9 am to 8pm on weekdays.
